New York State Licensure
You can obtain NYS License within just six-eight weeks, if you have a license from another state!
In order to get the NYS Board’s approval for a license several forms must be filled out and sent directly to the NYS Board from various terminals.
We briefly cover all the steps needed to obtain the NYS license bellow. For the official NYS Board website, please click this link.
Below are all the forms that need to be filled and sent to the NYS Board.
Each of the below forms are done and sent in separately.
Animal Hospital of Morris Park will cover all expenses to obtain your NYS licensure.

Form 1 – Application for License.

Fill out and send to NYS Board with applicable fees.

This form can be done electronically through the nysed.gov website.

Form 2 – Certification of Professional Education

Print and fill out Section I.

Send the form to your graduate school for them to complete Section II and send it to NYS Board.

The school must directly send the application otherwise the NYS Board will not accept it.

Form 2PPE – Certification of Pre-Professional Education

Print and fill out Section I.

Send the form to your undergraduate school for them to complete Section II and send it to NYS Board.

The school must directly send the application otherwise the NYS Board will not accept it.

The following 2 forms are only for veterinarians with licenses in other states who have not attempted the CCT (Clinical Competency Tests). If you have passed the NBE (National Board Examination) and CCT (Clinical Competency Test) or have tried to pass the CCT exam before or have passed the NAVLE, SKIP THIS SECTION.

Form 3- Verification Of Out-Of-State Licensure, Registration, And/Or Examination

Print and fill out Section I.

Send the form to the Board of whichever state you have your license in.

The state Board must directly send the application otherwise the NYS Board will not accept it.

Form 4 – Verification Of Professional Practice

This is a moral character form which can be filled out by any licensed veterinarian that can speak to your professional background.

Send the form to the professional veterinarian to complete and send to the NYS Board.

Relocation Process
After you receive your NYS License, we would proceed in the following manner:

We will request you to send us the following documents:

Your ID
-
Limited Permit/ NYS License

W2 / W9 form
-

We will buy you a ticket to come to NY.

You will come to our hospital to work one week as a Relief Veterinarian and see if our work atmosphere, values, and provided medicine meet your desires.

We will provide you with an apartment not far away from the hospital.

After we agree to work together, we will start the paperwork process in order to make you a permanent team member of Animal Hospital of Morris Park.

Accommodation will be provided for all full-time doctors from out of state.
